Childhood screen detects mental health risks

Children who experience traumatic events have a higher risk of health and mental health disorders. Traumas may include abuse, neglect, or domestic violence. But some children experience another type of adversity: unpredictable home environments. Examples include lack of a bedtime routine, parental mood swings, or an unstable relationship between parents.

Previous research has linked unpredictable parenting and home routines to poorer mental health in children. Yet, current screening methods don’t assess unpredictability during childhood. They are also better at identifying mental health conditions among large groups of people than for individual children.

An NIH-funded research team led by Dr. Tallie Z. Baram at the University of California, Irvine and Dr. Laura Glynn at Chapman University set out to address these limitations. The team added five unpredictability measures to a current adversity screen. They tested whether these measures helped improve the detection of childhood mental health conditions. 

The study included about 30,000 children seen in 19 pediatric primary care clinics in Orange County, California. The team compared mental health diagnoses from the children’s electronic health records with their screening results. The study appeared in Nature Mental Health on July 21, 2026.

The team found that the unpredictability measures correlated with mental health outcomes, performing as well as the traditional adversity screen. The unpredictability screen was better at identifying children at risk of developing depression and sleep disorders. For example, children who scored high in the unpredictability screen had a 12-fold higher risk of developing depression than a child who scored zero. Those who scored high in the traditional screen had an 8-fold higher risk of developing depression than a child who scored zero.

Combining the scores from both screens led to better detection of mental health disorders than either screen alone. Children who scored high on both screens were at higher risk of depression, anxiety, behavioral concerns, sleep disorders and physical symptoms, including headaches and abdominal pain.

The findings suggest that screening for unpredictability could identify children at risk for mental health conditions earlier. They could then be monitored and their families counseled before problems become more severe.

“Traditional screening has helped our understanding of childhood adversity, but it fails to identify many children who are at risk for mental health problems,” Glynn says. “Our findings suggest that unpredictability is a distinct and meaningful form of adversity that contributes to risk in ways that existing screening tools do not detect.”

“For parents, this isn’t about creating a perfect home,” Baram adds. “It’s about recognizing that consistency matters. Small routines and predictable patterns in daily life can support healthy development.”

Future studies are needed to understand how unpredictability can lead to mental health outcomes, and how to reduce its harms.
